Nancy Duzick, RN: How Horizon cut hospital readmissions by half

Transitional care offered for at least one month after discharge

— Nancy Duzick, RN, of Goshen spoke at a recent conference about how her team at Horizon Family Medical Group was able to cut hospital readmissions in half.

Director of Population Health and Transitional Care Management at Horizon, Duzick said this was accomplished through the team’s hospital visits to Horizon patients, timely phone contact with patients, and follow-up appointments with primary care physicians made promptly after discharge. Transportation or home visits are arranged as needed.

Horizon provides transitional care for at least a month after discharge. Duzick says the result has been greater patient satisfaction and engagement, improved health outcomes and quality of care, and better cost containment.

Duzick is a graduate of the University of Delaware. She has been at Horizon since 2014, when she and Eileen Bromm, RN, began the transitional care program. From 2012 to 2014, Duzick was nursing supervisor at Hospice of Orange and Sullivan Counties. She is certified in hospice and palliative care.

The conference was hosted by the New York Region’s Comprehensive Primary Care Plus (CPC+) program, in which Horizon participates. CPC+ is described on its website as “the largest-ever initiative of its kind to improve primary care in America.” It is sponsored by the national Center for Medicare and Medicaid Innovation.
